Itachi reveals it all in this chapter with a bit of story-telling, of two brothers who were born with the gift of the Mangekyou Sharingan. But cursed with blindness, the elder brother stole the eyes of his younger brother, thus gaining permanent usage of the Mangekyou Sharingan minus the blindness side-effect.
With this knowledge, Itachi intends to harvest Sasuke’s eyes for himself.
Sasuke, fueled with this revelation, isn’t going to surrender his organs that easily. But what can he do against his brother’s power of the Mangekyou?!
